What it is

Gonadorelin is best known for telling the pituitary to release the hormones (LH and FSH) that drive testosterone production and fertility. It's a synthetic copy of the body's own gonadotropin-releasing hormone (GnRH), a ten-amino-acid peptide. It was FDA-approved historically (as Lutrepulse and Factrel), but both products were withdrawn from the market for commercial reasons, not safety. Today it's available in the US only through compounding pharmacies, used off-label mainly as a testosterone-therapy adjunct to keep the body's own hormone signaling active.

What the research found

Gonadorelin has been studied to stimulate the reproductive-hormone axis, and its effect depends entirely on timing — pulsed dosing stimulates the axis while continuous dosing suppresses it. The strongest evidence (older fertility-induction studies and a 2019 trial where a pulsatile pump induced sperm production in most men with a hormone deficiency) used precise pump-delivered pulses, not the once- or twice-weekly injections common in research-community use — so that common off-label pattern is not directly supported by the trial evidence.

Status and regulatory position

FDA-approved historically as Lutrepulse (Ortho Pharmaceutical / R.W. Johnson, NDA approval late 1989) for primary hypothalamic amenorrhea (anovulatory infertility from GnRH deficiency in women) — pulsatile IV administration via the Zyklomat / OmniPod pump. Also FDA-approved historically as Factrel (Wyeth-Ayerst) for diagnostic GnRH stimulation testing in evaluation of pituitary gonadotropin reserve. Both products were withdrawn from the US commercial market for commercial reasons (not safety or effectiveness). Currently available exclusively via 503A compounding pharmacies in the US for off-label use — primarily as a TRT adjunct (testicular function maintenance during testosterone replacement therapy) where it has gained substantial popularity driven by the March 2020 FDA reclassification of HCG from a drug to a biologic (which ended 503A compounded-HCG supply), compounded by the 2021–2023 retail-HCG shortages.[¹] not DEA-scheduled. WADA-banned in regulated sport — explicitly named under Section S2.2.1 (Testosterone-stimulating peptides in males) of the 2026 Prohibited List, alongside HCG, LH, and the GnRH agonist analogues (buserelin, deslorelin, goserelin, histrelin, leuprorelin, nafarelin, triptorelin) and kisspeptin.[⁶]

⚠️ Precision matters at low doses. Gonadorelin is dosed in micrograms (mcg), not milligrams. Typical research-community / TRT-adjunct doses ( per dose) at typical reconstitution concentrations ( vial / 1 mL BAC water =) produce small draws in the range on a U-100 syringe. At those draw sizes, even a one-unit error is a meaningful percentage of the dose. Always verify draws against the in-app calculator. The precision concern is comparable to BPC-157, GHK-cu, mt-II, and igf-1 lr3.

⚠️ Pulsatile vs continuous administration produces opposite effects — this is the load-bearing pharmacological detail. Gonadorelin's mechanism critically depends on the administration pattern: - Pulsatile administration (every 90–120 minutes, mimicking endogenous GnRH pulse pattern) → physiologic LH and FSH release → testicular function stimulation in men, ovulation induction in women. This is the original Lutrepulse FDA-approved use case (delivered via Zyklomat / OmniPod portable infusion pump for primary hypothalamic amenorrhea). - Continuous administration or single high-dose administration → GnRH receptor downregulation/desensitization → suppression of LH and FSH release → suppression of testicular function. This is the basis for the FDA-approved GnRH agonist drugs (leuprolide / Lupron, triptorelin, goserelin) used for prostate cancer, endometriosis, central precocious puberty, and GnRH agonist-suppression contexts. Research-community TRT-adjunct dosing (typically SC 2–3× weekly) sits between these extremes — frequent enough to provide meaningful LH/FSH stimulation, but not as frequent as the every-90–120-minute pulsatile pump that produces the strongest physiologic response. The published clinical evidence for fertility induction (Lutrepulse, congenital hypogonadotropic hypogonadism trials) used true pulsatile-pump administration; the research-community 2–3×-weekly dosing pattern is a practical compromise that does not directly correspond to the published trial regimens. Researchers using gonadorelin for TRT-adjunct purposes should be explicit with themselves about this evidence-base / dosing-regimen mismatch.

⚠️ The "HCG-alternative" use case context: a 2021–2023 supply-disruption-driven shift, not a fully evidence-equivalent substitution. Gonadorelin's recent surge in research-community popularity is largely driven by HCG supply disruptions that began in early 2021 and persisted through 2023 (regulatory and manufacturing issues affected HCG availability through US retail and compounding pharmacies). Compounding pharmacies offered gonadorelin as a substitute because (a) gonadorelin is on the FDA approved-API list and is legally compoundable under 503A; (b) it stimulates the same downstream LH-driven testicular function that HCG stimulates, just upstream (at the pituitary, vs HCG's direct LH-receptor agonism at the testes). However, the substitution is mechanistically and clinically distinct, not equivalent: HCG directly stimulates Leydig cells (bypasses the pituitary entirely); gonadorelin works through the pituitary and depends on intact pituitary somatotroph function. Patients with pituitary dysfunction will not respond to gonadorelin but will respond to HCG. Subjective patient reports are mixed — some research-community sources describe gonadorelin as "providing comparable benefits" while others report patients on gonadorelin "have not felt the desired effects." Researchers selecting gonadorelin specifically for fertility preservation during TRT should weigh that the strongest published clinical evidence (Zhang 2019, congenital hypogonadotropic hypogonadism cohorts) used pulsatile-pump administration, not the 2–3×-weekly research-community pattern.[⁵]

⚠️ WADA-banned in regulated sport — explicitly named under Section S2.2.1 (Testosterone-stimulating peptides in males). The 2026 WADA Prohibited List Section S2.2.1 names *"gonadotrophin-releasing hormone (GnRH, gonadorelin) and its agonist analogues (e.g. buserelin, deslorelin, goserelin, histrelin, leuprorelin, nafarelin and triptorelin)"* alongside chorionic gonadotrophin (CG / HCG), luteinizing hormone (LH), and kisspeptin and its agonist analogues.[⁶] This is the same WADA subsection as HCG — the S2.2.1 framework covers all peptides that stimulate testosterone production via the upstream HPG axis. Prohibited at all times, in-competition and out-of-competition. Note the regulatory irony: compounding pharmacies are offering gonadorelin specifically because kisspeptin is not approved for compounding under California state regulations, while gonadorelin IS — but both compounds sit in the same WADA S2.2.1 category and are equally prohibited in regulated sport.[⁶]

In depth

Gonadorelin is synthetic gonadotropin-releasing hormone (GnRH), identical in amino acid sequence to endogenous human GnRH (also called LHRH — luteinizing hormone-releasing hormone). The decapeptide sequence is pyroGlu-His-Trp-Ser-Tyr-Gly-Leu-Arg-Pro-Gly-NH2 (molecular weight 1182.33 Da). GnRH is the master regulator of the hypothalamic-pituitary-gonadal (HPG) axis — endogenous GnRH is released from the hypothalamus in pulses approximately every 90–120 minutes, traveling through the hypophyseal portal system to bind GnRH receptors on pituitary gonadotroph cells, triggering pulsatile LH and FSH release. The downstream LH and FSH then stimulate gonadal function (testosterone production and spermatogenesis in men; follicular development and ovulation in women).

Mechanism — pulsatile vs continuous administration produces opposite effects. This is the load-bearing pharmacological detail of GnRH biology: - Pulsatile administration (every 90–120 minutes, mimicking endogenous physiology) → maintains GnRH receptor responsiveness → triggers physiologic LH and FSH release. This is the basis for fertility-induction use (Lutrepulse for primary hypothalamic amenorrhea; pulsatile-GnRH-pump for spermatogenesis induction in congenital hypogonadotropic hypogonadism). - Continuous administration or sustained-release agonist administration → progressive GnRH receptor downregulation and desensitization → suppression of LH and FSH release → suppression of gonadal function. This is the basis for the FDA-approved GnRH agonist drugs — leuprolide (Lupron / Eligard), triptorelin, goserelin (Zoladex), buserelin, nafarelin (Synarel), histrelin, deslorelin — used for prostate cancer hormonal therapy, endometriosis suppression, central precocious puberty, and gender-affirming care contexts.

Gonadorelin's short half-life (~2–4 minutes plasma) is the principal pharmacological constraint that determines its use cases. Sustained LH/FSH stimulation requires either: (a) pulsatile-pump administration (the original FDA-approved Lutrepulse approach), or (b) frequent injection (the practical research-community approach for TRT-adjunct use, which is a compromise from the more clinically validated pulsatile-pump protocols).

FDA regulatory history. Two distinct gonadorelin products received FDA approval historically:

- Lutrepulse (gonadorelin acetate) — Ortho Pharmaceutical / R.W. Johnson, FDA-approved late 1989 for treatment of primary hypothalamic amenorrhea (anovulatory infertility from GnRH deficiency in women) via pulsatile IV administration using the Zyklomat / OmniPod portable infusion pump. The product enabled physiologic ovulation induction in women with hypothalamic GnRH deficiency. Withdrawn from the US commercial market — the complexity of pulsatile-pump administration limited adoption to specialized fertility practices, and the indication is rare; Lutrepulse was discontinued for commercial reasons.[¹] - Factrel (gonadorelin hydrochloride) — Wyeth-Ayerst, FDA-approved for diagnostic GnRH stimulation testing to evaluate pituitary gonadotropin reserve. Used as a diagnostic agent to differentiate hypothalamic from pituitary causes of hypogonadism. Also discontinued from the US commercial market.

Current US regulatory framework (compounding-pharmacy supply). With both branded products withdrawn, gonadorelin is currently available in the US exclusively via 503A compounding pharmacies (Empower, Strive, Clearwater Aesthetics, and others) for off-label use. This is the same regulatory category as Sermorelin — FDA-approved historically, voluntarily withdrawn from US commercial market for commercial (not safety) reasons, currently legally compoundable under 503A. Notably gonadorelin is not subject to the recent 503A bulks list controversy that affected several research peptides (CJC-1295, BPC-157, TB-500, AOD-9604, ipamorelin, thymosin α1) — gonadorelin remains on the FDA approved-API list and is legally compoundable. Researchers obtain gonadorelin through compounding pharmacies under prescription, dispensed as or solutions in 2 mL or 5 mL multidose vials.

Why gonadorelin's research-community popularity surged. A confluence of factors drove a dramatic increase in research-community gonadorelin use over the past several years:

1. The March 2020 FDA reclassification of HCG from a drug to a biologic was the structural driver — it permanently removed HCG from 503A compounding eligibility, leaving only a handful of 503B outsourcing facilities able to make it. Retail-HCG shortages that began in early 2021 and persisted through 2023 (affecting retail-pharmacy HCG such as Pregnyl and Novarel) layered on top of that structural change. TRT clinics that had been routinely prescribing HCG alongside testosterone for testicular function maintenance / fertility preservation suddenly could not reliably source HCG. 2. Compounding pharmacies pivoted to gonadorelin as a substitute — gonadorelin remained on the FDA approved-API list, was legally compoundable, and had a plausible mechanism for the same TRT-adjunct goals (LH/FSH stimulation → testicular function maintenance). 3. Major TRT clinics began promoting gonadorelin as an HCG alternative — Empower Pharmacy, Strive, Defy Medical, and others added gonadorelin to their TRT-adjunct compounded medication offerings. 4. The shift accelerated — by 2023, many TRT clinics were prescribing gonadorelin by default for new TRT patients, even after HCG supply began recovering, because the compounded-gonadorelin supply chain had become routine.

Common research interests. Despite the regulatory complexity, gonadorelin is widely used in research-community contexts for: - TRT adjunct — fertility preservation / testicular function maintenance. Co-administered with testosterone replacement therapy to maintain endogenous testicular function, prevent testicular atrophy, and preserve fertility. The dominant research-community use case. - HPG-axis recovery during or after androgen cycles ("post-cycle therapy" / PCT contexts). Used to restore HPG-axis function after exogenous androgen suppression. Often combined with SERMs (clomiphene, tamoxifen). - Pulsatile-pump fertility induction in congenital hypogonadotropic hypogonadism — published RCT evidence supports this use; not common in the research-community / off-label context but is the strongest evidence-base use case. - Diagnostic GnRH stimulation testing — historical use, now largely supplanted by other diagnostic approaches.

Reported side effects

Commonly reported

  • Injection site reactions — most common reported effect; local pain, redness, mild swelling. More common with frequent dosing (the research-community 2–3× weekly pattern).
  • Hypersensitivity reactions — uncommon but reported; range from mild local reactions to (rare) anaphylactoid responses. The pulsatile-pump administration historically had a small but documented anaphylactoid risk.
  • Headache — uncommon
  • Nausea, abdominal discomfort — uncommon
  • Hot flashes — uncommon
  • Mild flushing — uncommon
  • Subjective response variability — some users report meaningful TRT-adjunct effects (testicular function maintenance, libido changes); others report no discernible effect. This variability is consistent across multiple research-community sources and is the most-cited concern about gonadorelin's substitution for HCG.
  • No consistent serious-adverse-event pattern in the published literature or accumulated compounding-pharmacy use experience
  • Long-term safety in adults using gonadorelin for off-label TRT-adjunct purposes is not characterized in published clinical literature. The FDA-approved Lutrepulse program had defined endpoints in women with primary hypothalamic amenorrhea; multi-year off-label TRT-adjunct use in men is not characterized.
  • Pituitary gonadotroph desensitization with frequent or high-dose administration — theoretical concern; the line between "frequent enough to stimulate" and "frequent enough to desensitize" is not precisely characterized in the off-label TRT-adjunct dosing range.

Regulatory note (US): gonadorelin is available via compounding pharmacies only in the US currently (FDA-approved branded Lutrepulse and Factrel both withdrawn). Off-prescription or non-compounded supply is illegal under federal law.
